POC Workflow Email Process

Walkthrough: Sending and Certifying Plan of Care (POC) via Email

The updated POC workflow streamlines how clinics send Plans of Care to referring physicians, who can now review, sign, or request revisions directly from their email. Follow these steps to use the new email-based POC certification process.

 

Step 1: Complete the swap node in your workflow. Once the POC is generated, look for the Email POC to Certify option.

Step 2: Click Email POC to Certify. The referring physician will receive an email with a secure link to the dashboard.

Step 3: After sending, the POC appears in the Send for Certification tab within your task dashboard.

Step 4: The referring physician opens the email and clicks Review and Sign Plan of Care.

Step 5: Upon verifying credentials, the physician can view the POC on their dashboard. They can choose one of three actions:

  • Sign and Certify the POC immediately.
  • Send for Revision if changes are needed (with a reason).
  • Postpone signing and return to the dashboard later.  

Step 6: If sending back for revision, document the reason and click Send for Revision.

Step 7: To sign the POC at a later time, return to the dashboard, select the pending POC, and finish the certification process.

Step 8: The dashboard clearly displays:

  • POCs pending signature
  • Certified POCs
  • Sent for revision POCs  

Step 9: To review certified POCs, click View Document.

Step 10: To sign a pending POC, click Review and Sign, then choose Sign and Certify. The physician’s signature appears, and they can save and certify the document.

Step 11: Once signed, the POC moves into certified status on the dashboard, and is also automatically certified in the patient’s POC tab.

Certified POCs are immediately updated across both the physician and clinic dashboards. Manual certification is no longer necessary once the referring physician completes the process.

This workflow makes it faster and easier to share, sign, and track Plans of Care, reducing delays and improving communication between clinics and referring physicians.
